Casino Video/Film Permission Application
Please complete the application in full. All fields are required unless marked optional, and submission does not guarantee approval — we review every application and respond by email or phone.
Submit your completed form to our Marketing Manager, Harold, at HHernandez@HialeahPark.com. We aim to respond within 5–7 days.
Agreement & Stipulations
By signing the application you agree to the following terms for filming and vlogging at the casino.
Content Review
All videos must be submitted for review and approval in writing by casino management before they are posted or published online.
Content Removal
If management requests removal of a video or post, it must be taken down as soon as possible. Non-compliance may revoke filming permission.
Conduct & Consent
Follow all casino rules: obtain consent from other players or guests before filming them, avoid disrupting games, and comply with staff instructions.
Revocation
Permission to film may be revoked at any time at the discretion of casino management.
English only can be spoken at gaming tables and machines unless otherwise agreed upon.
